Open communication is important to Dr. Tran Le. Current and former clients are encouraged to contact Dr. Tran Le directly with any questions, concerns, or requests related to services, records, billing, scheduling, or this notice.
Clients may contact Dr. Tran Le by email, phone, secure client portal message, or the website contact form. Messages received during business hours, excluding weekends, holidays, and provider leave, will typically be returned within 48 business hours.
If a client does not receive a response within that timeframe, the client is encouraged to follow up and try a different communication method, such as email, phone, or the secure client portal. Occasionally, technology issues, message delivery problems, or other delays may occur, and following up helps ensure the message is received.
If a client has an urgent mental health concern, is in crisis, or is in immediate danger, the client should call 911, go to the nearest emergency room, or contact the 988 Suicide & Crisis Lifeline.
Notice to Consumers
Access to Health Care Records and Consumer Complaints
Effective September 1, 2025, Texas Health and Safety Code § 181.105 requires covered health care entities to prominently post instructions for consumers to request health care records, contact the appropriate licensing authority, and file a consumer complaint. This notice applies to services provided by Dr. Tran Le, LPC, Reflective Space Counseling and Consulting PLLC.
Requesting Your Health Care Records
You have the right to request a copy of your health care records. To request records from Reflective Space Counseling and Consulting PLLC, please submit a written request by email or through the secure client portal.

Records will be provided in accordance with applicable Texas law and federal privacy regulations. Some records may be limited, withheld, or released in a specific format as allowed or required by law.

If you have a concern about services received, you may contact Dr. Tran Le directly by phone, email, secure client portal message, or the website contact form so the concern can be reviewed and addressed.
You also have the right to file a complaint with the appropriate licensing or consumer protection agency.
